Transition Professionals is a 501(c) (3) nonprofit organization dedicated to supporting individuals as they reenter the workforce and their communities. Our mission is to provide holistic, trauma-informed, and people-centered services that empower participants to thrive, reduce recidivism, and achieve sustainable employment and stability. 

Our services are delivered through four key programs: 

The Core Program offers case management, community support, and guidance to Bergen County residents navigating justice involvement. Through personalized support, we assist clients in accessing vital documents, applying for benefits, assisting with housing, and connecting to mental health, substance use, and social services. 

The JOBS Program (Job Opportunities for Building Success) provides justice-involved individuals in Bergen, Passaic, Essex, Hudson, Morris, Sussex and Warren county with comprehensive workforce development, including tailored job readiness training, vocational certifications such as ServSafe, OSHA-30, and CDL, resume building, and job placement support. Participants also receive ongoing retention support and access to essential resources, including transportation and professional clothing. 

The HOPE Program (Helping Opioid-affected Populations through Empowerment) addresses the needs of Bergen County residents impacted by opioid use disorder. This program provides recovery-oriented, harm-reduction services, trauma-informed support groups, overdose prevention education, and individualized recovery planning to promote long-term stability and reentry success. 

The LEAD Program (Locally Empowered, Accountable, and Determined) expands community-based reentry services, supporting participants in Bergen, Passaic, Essex, Morris, Sussex, and Hudson County who have been released from a northern state prison within the last 7 years. LEAD participants receive access to benefits, housing, job readiness, and referrals to local social and justice-related services. 

Together, these programs create a comprehensive framework for reentry and workforce development. Transition Professionals empower participants to overcome barriers, achieve goals, and build meaningful, sustainable futures.
